This Agreement is entered into between Veterinary LinQ LLC, a Texas limited liability company ("Vet LinQ"), and you as a representative of the clinic described above:
Purpose
This authorization grants Vet LinQ permission to access and retrieve patient medical records from your clinic's Electronic Medical Records (EMR) system. This access is solely for the purpose of fulfilling pet owner requests for record sharing with other veterinary facilities and improving continuity of care.
Authorization of Access
- Clinic grants Vet LinQ permission to access and retrieve patient medical records upon verified owner request.
- Authorization is for data retrieval only and does not permit sharing, selling, or distributing data to third parties.
- Access via secure API, encrypted data transfer, or other agreed technical means.
Data Security & Compliance
Vet LinQ agrees to maintain industry-standard security and privacy controls, including:
- End-to-end encryption (AES-256).
- OAuth 2.0-based access tokens and scoped authorization.
- Zero-trust architecture with least-privilege access.
- Third-party vulnerability and security audits.
Data Retention & Use
- Records accessed will be read-only; no alteration or editing.
- Vet LinQ will not store medical records retrieved from the clinic.
- Vet LinQ may retain limited metadata and analytics (e.g., access logs, record hashes, audit entries) for security, compliance, and performance.
Revocation & Termination
- Clinic may revoke authorization at any time by written notice to support@vetlinq.com.
- Upon revocation, Vet LinQ will disable access and confirm termination in writing.
- Either Party may terminate with or without cause upon written notice.
Representations
Clinic represents authorization to execute the agreement; Vet LinQ represents compliance with laws, ethical standards, and confidentiality obligations.
Governing Law
Agreement governed by Texas law.
Entire Agreement
Document constitutes the entire understanding; modifications must be in writing and signed by both Parties.
